School staff will be present to monitor student drop-off and pick-up 15 minutes before and after the school day begins and ends. Students dropped-off earlier or picked-up later than these times should be enrolled in the Before/After School Program.
PLEASE NOTE: ALL STUDENTS ON CAMPUS BEFORE 7:45 A.M. AND AFTER 3:15 P.M. MUST BE IN A SCHOOL SPONSORED ACTIVITY OR IN CLUB HORIZON. CLUB HORIZON
The Club Horizon Program is offered before and after school for students in Pre-K to grade 6. Students not in attendance on the Horizon Honors campus are not eligible to attend Club Horizon. Club Horizon is located at the east end of the campus in the elementary wing. Entrance to the program is through the gate next to the activity fields; walk north along the building to the second entrance. The entrance is clearly marked.

A $25.00 Late Fee is added to the Vouchers if not paid by the due date.
Scheduled Payments:
These are Voucher payments that are paid online by on the due date by midnight.
Credits:
Credits are not available. If you make previous commitments for attendance, you are expected to use the days specified.
Drop-In:
A Drop-In day is for emergency use only. You may use this service if your student does not attend Club Horizon Regularly. The Late Fee does not apply for a Drop-In. The fee for services tendered is due at time of pick-up.
CAMP HORIZON
The Camp Horizon Program is provided for students entering or in grades Pre K-6 during the Fall, Winter, Spring, and Summer Breaks. Camp Horizon is not open on scheduled Holidays. The camp meets daily except on legal holidays from 6:15 A.M. to 6:00 P.M. Camp Horizon is located at the east end of the campus in the elementary wing. ENROLLMENT PROCEDURE
Club Horizon is offered to any child who is enrolled in Horizon Honors Elementary School or PreK at Horizon Honors. Students not in attendance at Horizon Honors Elementary School or PreK at Horizon Honors are not eligible to attend Club Horizon. The program utilizes a voucher system. This voucher must be completed by the parent/guardian and is child specific. These vouchers define the program offered (Before school, After school or Camp Horizon) and the days scheduled. A parent/guardian is required to complete the voucher online located on the Horizon Honors Elementary School website prior to services. All vouchers are date specific and have due dates listed. Any student attending Club Horizon for the first time must contact the Club Horizon office and complete the appropriate voucher and emergency contact form prior to attendance.
All students must have the Arizona Immunization Record (ASIR109R) on file and updated in the main health office to be enrolled in the school. We have additional access to emergency contact information during Club Horizon hours.
REFUND POLICY, PAYMENT POLICY
All fees for services are NON-REFUNDABLE. Vouchers are due one week prior to services and are used to determine staffing and supplies. All vouchers can be printed off of the Horizon Honors Elementary School Website under Club Horizon, they can also be picked up in the front office as well as in the Club Horizon office.
ARRIVAL & DEPARTURES
A parent/guardian is required to sign their children in and out of the program upon arrival and/or departure. Club Horizon staff will sign your child/children out in the morning session and release them to their classroom. For the afternoon session, the assigned Club Horizon staff will sign your child/children in at the time of attendance. You as the parent/guardian are responsible for signing your child/children out of the program as you leave the facility. Due to our governing policies, you as the parent/guardian must sign your first initial and last name. This is a DHS requirement. Please do not let your child leave ahead of you. Your cooperation will ensure the safety of your child.
If you wish for someone other then those listed on your emergency information card to pick your child up, you must notify the Director in writing or by telephone. Each person picking up your child/children must be prepared to show picture identification. Without proper authorization or identification, we will NOT release your child/children. This is for the safety and well being of your child/children.
The facility closes at 6:00 P.M. sharp! A late fee of $1.00 per minute will be assessed for pick-up after 6:00 P.M. This payment is due at time of pick-up. If for any reason an emergency arises and you will be late for pick-up, please notify the CLUB HORIZON office by calling (480) 659-3182.
HOMEWORK
In Club Horizon we designate time every day for homework help or academic improvement. During this time, students are required to focus on their studies. Students will be monitored during Homework Help time. Some students may finish their assigned work during this time. Please review completed assignments with your child each evening.
GUIDANCE AND COUNSELING SERVICES
All staff members are trained in problem solving which serves as our first step in resolving conflict and providing emotional support. Students whose needs cannot be met by this immediate support will be assisted by Horizon Honors Administration or our Student Advisor. Other community resources will be secured as the needs arise. Parent/guardians are asked to contact their student’s classroom teacher, Master Teacher or Principal with specific needs or concerns.
GUIDELINES FOR INAPPROPRIATE/FORBIDDEN ITEMS
Students are not allowed to have the following items at school: aerosol cans, alcohol, balloons, bandanas, beepers/pagers, cellular phones, cigarettes, “discmans”, “do-rags”, drugs, eggs, glass containers, firecracker/works, gum, permanent markers/pens, personal alarms, pornography, pepper spray/mace, shaving cream, “silly string”, “slam” books, snap caps, smoke/stink bombs, inappropriate stickers, sunflower seeds, toys, “walkmans”, water balloons, water guns, weapons, any chains including key and wallet chains, tie wraps, and any other questionable items.
These items will be confiscated and must be picked up by a parent/guardian. Students will be assigned appropriate consequences for possession of these items in accordance to the Student Code of Conduct.
Inappropriate use of the following items is not allowed: Correction fluid, glue, paint, paper clips, rubber bands, scissors, staples, and felt markers/pens, etc.
We do not allow CD players, gameboys, game consoles, ipods, mp3 players, or cell phones during breaks.
TERMINATION OF SERVICES
The behavior expectation (code of conduct) is defined in this Handbook. We use problem-solving techniques to correct any behavioral issues, however, CLUB HORIZON reserves the right to dismiss a child under the following conditions:
1. Delinquent tuition.
2. Due to adverse behavior of a child that has a negative effect upon other children and/or the staff’s ability to administer the program.
3. Destruction of Property.
4. When the behavior of the child and/or the parent/guardian, affects the mental and physical safety of the other children and/or the staff.
5. Abusive and/or foul language from child and/or parent/guardian.
6. Incompatibility with children and/or the staff.
7. Lack of parent/guardian cooperation in adhering to CLUB HORIZON’s policies.
8. When it is determined by the Director and/or management, that CLUB HORIZON is not equipped or staffed to deliver meaningful and/or safe services to a specific child and family.
HEALTH SERVICES POLICIES
A parent/guardian is responsible for picking up their child or making arrangements for an emergency contact to pick up students who are ill or injured.
A parent/guardian is required to complete an emergency information form for each child at the beginning of each school year and notify the office staff of any changes as soon as possible throughout the school year.
If both parent/guardians will be out of town, or unavailable for contact, a letter designating the person whom will be responsible in case of an emergency must be on file with Health Services and with Club Horizon.
Parent/guardians of children with serious health problems (i.e. epilepsy, diabetes, hemophilia, severe asthma or allergy, or other special needs) are requested to report this information to Health Services and provide special health care instructions.
Any student with a temperature of 100.0 degrees or higher may not attend school. Children may not return to school until they are fever-free for twenty-four hours. Students who are vomiting, or having diarrhea, will be sent home.
To minimize exposure to other children, please consider keeping your child home if you see these symptoms. If symptoms persist, you may choose to contact your doctor.
1. Fever of 100.0 degrees and above. Child must be fever-free for twenty-four hours before returning to school.
2. Persistent cough
3. Severe sore throat
4. Rash with fever illness, such as chicken pox, measles, etc.
5. Nausea, vomiting, or diarrhea
6. Red, itchy or burning and draining eyes - if conjunctivitis or "pink eye" is diagnosed, the child must be on medication for twenty-four hours before returning to school
7. Prolonged headache, stomachache, earache, or toothache
8. Swelling or pain at a level that may interfere with ability to concentrate and learn
9. Head lice - child must remain at home until treated with medicated lice shampoo and all the nits are removed. Please notify Health Services if lice have been identified
MEDICATIONS
• Club Horizon assigned personnel will administer over-the-counter and prescription medication during after school hours in accordance with the Arizona State Board of Pharmacy.
• Whenever possible, medications should be given at home.
• There must be a written order from the physician stating the name of the student, name of the medication, dosage, frequency, time and the reason it is to be given.
• Written permission from the parent/guardian for medication to be given at school must include the completion of an "Administering Medications to Students" form, available from Health Services. Written notes from home will not be accepted.
• Medications are not to be sent to school with students. All medications, including daily prescription medications, refill, over-the-counter medications, and as needed medications, must be brought to the Club Horizon office by the parent/guardian.
• Prescription medication, including refills, must be in the original prescription container labeled by the pharmacist.
• Over-the-counter medication must be in the original container with all warnings and directions intact.
• Prescriptions and precautions must be in English.
• Club Horizon will not give medication beyond the recommended dosage or frequency listed on the container or the doctor's order, even if requested by the parent/guardian.
• Medications must be current. Club Horizon will not give medications past the expiration date on the label.
• Only medications needed to treat an existing ailment will be stored at school. All other medications must be picked up by the parent/guardian and returned home.
• Acetaminophen (Tylenol) will be given only to students who have written permission from a parent/guardian indicated on the emergency information form. Acetaminophen will be given as needed within the guidelines and directions listed on the label, for a maximum of three days in a row.
• After three consecutive days, Club Horizon must have a written order from a physician to
continue giving the medication. Acetaminophen cannot be given to reduce fever; if a fever is present the student must be picked up from the program within the appropriate time.
• Aspirin will not be given at school unless prescribed in writing by a physician.
• Club Horizon will not give out cough drops.
• Inhaler medications must be in the original prescription box labeled by the pharmacist. As needed (PRN) inhalers will be given as directed on the prescription. Students will be assessed for symptoms that indicate a need for the medication (i.e. wheezing, cough, chest tightness, difficulty breathing).
• A written physician’s order is needed to give inhalers on a daily or scheduled basis (i.e. everyday before PE or daily at 10 am and 2 pm), or more frequently than written on the prescription label.
• Medication for SVN treatments must be in the original box with the pharmacy label attached. If box is not available, we must have a written doctor's order or faxed doctor's order for the medication.
• Per parent/guardian request, medication will be given via SVN on a scheduled daily basis for 3 days in a row only. After three days, we must have a written doctor's order or faxed doctor's order to continue giving the medication on a daily basis.
• Self-administration by a student of inhaler medications will only be allowed when a physician feels it is necessary and provides written recommendations and instructions. These recommendations will be attached to the permission form indicating that the student has been instructed in the proper use of this medication and the parent/guardian feels the student is responsible to use it as directed by the physician. Both the parent/guardian and student will sign this form. Horizon Honors reserves the right to circumscribe or disallow the self-administration of medication on school premises.
• Doctor's orders must be in writing on physician letterhead or script. Doctor’s orders may be faxed to the office at 480-659-3022, attention: Health Services, specify students full name and whether they attend elementary, middle or high school.
CHICKEN POX
Every student must remain home the minimum of seven to ten days after the first pox appear, and the pox must be completely scabbed over before the student returns to school. The student must check in with the health coordinator the day he or she returns to school.

As you read through this lengthy handbook of Parent POLICIES AND PROCEDURES, please be cognizant of the fact that we are licensed by The Arizona Department of Health Services (DHS) and monitored for compliance by several other agencies. Everything we plan and implement is for either/or the health and safety of “our” children, developmentally appropriate, based upon the latest research, and/or is a requirement of one the governing agencies.
